Community Health Worker

Community Health Worker services can help with issues that affect people every day, such as the need for preventative treatments, behavioral health issues, and the management and prevention of infectious infections or chronic disorders. Community Health worker services can also assist medical members in accessing appropriate services for perinatal care, sexual and reproductive health, aging, injury, oral health, environmental and climate-sensitive health issues, domestic violence, and community violence prevention.

Duties

Building relationships within a community to connect people with healthcare services, educating people about health issues that are relevant to their culture, advocating for community needs, screening, referring and navigating social service systems to address social determinants of health are the main goals of a community health worker (CHW), frequently through outreach programs and home visits.

Career Opportunities

Community health advocates, navigators, and other unlicensed public health professionals, such as violence prevention specialists, are examples of people who may be referred to as Community Health Workers.

Day in the Life

Community Health Workers (sometimes called Case Managers, outreach coordinators) make it a daily priority to assist and support patients in every manner they can to help them deal with the difficulties they encounter. Supporting a patient during pregnancy, giving families access to resources they were unaware of, or helping patients obtain health insurance for themselves and their family are examples.
